What is Laparoscopy Keyhole Surgery?

Keyhole surgery, also known as laparoscopic surgery, involves making two or three tiny incisions through which a small camera (laparoscope) and surgical instruments are inserted. This allows our veterinary surgeons to perform precise procedures with minimal trauma to the surrounding tissues.





What is a laparoscopic spay and the difference between a traditional spay?

Both traditional and laparoscopic surgeries remove the reproductive organs of a female dog which is mainly performed to prevent the them having puppies, but also have other health benefits such as the prevention of a pyometra (infection of the uterus) and reduces the risk of developing mammary and other reproductive organ tumours. 


In a traditional spay (open ovariohystorectomy) the surgeon makes a large 2-4 inch incision in the midline of the abdomen, then the ovaries and uterus are identified, ligated (sealed) and removed from the body. The three layers of the abdomen are then sutured closed. 


In a laparoscopic spay (keyhole ovariectomy) the surgeon makes two small incisions of the abdomen roughly 1-1.5cm in size. Gas is then used to inflate the abdomen and cameras are inserted into the abdomen through ports. The surgeon identifies the ovaries, detaches them with a cautery (a instrument that cuts and burns tissue) and removes them throught the small hole previously created. The small incisions are then sutured closed. 


Advantages of a laparoscopic spay


  • Improved visualisation and precision: Using cameras gives an enhanced view of the abdomen allowing for a more precise procedure and allows the surgeon to more accurately identify any bleeding. 


  • Minimally invasive and faster recovery: smaller incisions and less tissue manipulation lead to a less painful procedure and a faster recovery when waking up from anaesthetic and in the weeks after your dogs surgery. On average time until completely healed in lap spays is halved compared to a traditional spay from 2 to 1 week. 


  • Reduced risk of complications during and post surgery: laparoscopic spays have a lower risk of haemorrhage as less vessels need to be ligated. The use of a cautery also allows more accurate ligation of these vessels. Smaller incisions and less suture material used will reduce the risk of post operative infections. 


  • Reduced scarring: smaller incisions lead to faster healing and less noticeable scars after surgery 


Disadvantages of a laparoscopic spay 


  • Conversion to a traditional spay: Occasionally the surgeon may decide to convert to a traditional spay if abnormalities are identified or complications arise, e.g., an abnormal uterus or uncontrolled haemorrhage. 


  • Cannot be used if there is any chance of uterine abnormalities or infection present. 


  • Ovarian remenants: very rarely if part of an ovary is not fully removed hormones are still created which can still affect reproductive organs and cause complications later in life e.g. a pyometra.


  • Longer surgical time: surgery can sometimes take longer due to equipment set up. 


  • Increased cost: due to the use of more specialist equipment.


  • Larger fur clip: as cameras are inserted into both sides of the abdomen a larger aseptic field is required.


Damage to other organs: Very rarely equipment may cause damage to other abdominal organs e.g. burn damage to surrounding tissue.
